Nutrition Education CHW

Position in Philadelphia, Pennsylvania
Position Description:
New Kensington Community Development Corporation's Nutrition Education team is looking to bring on a community health worker to assist in facilitating classes and workshops as well as the organization's community outreach efforts. The CHW will work with the Nutrition Education Managers to help facilitate nutrition education programming, build relationships with community-based food distributors and partner organizations and support Policy, systems and environmental changes in Kensington. Programming entails providing hands-on or web-based lessons and workshops while addressing specific areas of expertise such as nutrition, healthy cooking, exercise, food budgeting and goal setting using our standardized curriculum. Additional training and guidance will be provided on the job, and there is an option to complete a certificate in nutrition as well.
Position Requirements:
Applicant must be comfortable standing for extended periods during events and will likely be asked to occasionally assist in setup that requires lifting as much as 20 pounds at a time.
Applicant should be comfortable interacting with many members of the community as well as with public speaking.

Community Health Worker
Latino Connection
Position in Harrisburg, Pennsylvania
Position Description:
Latino Connection is a public health nonprofit providing health outreach and education resources to underserved communities. The dedicated staff at Latino Connection intimately understand the unique values, motivators, and concerns of Latino communities and work to empower and uplift the most vulnerable – and wonderfully vibrant- communities. They offer a variety of services, ranging from mobile COVID testing vans to nationwide webinars and community outreach events to raise awareness on menthol tobacco. They are seeking two already trained Community Health Workers (CHWs) to assist in some of their current programs.
The CHWs will support the Blood Pressure Self-Monitoring Program (BPSMP). They will play a vital role in helping participants manage their blood pressure by providing guidance, education, and ongoing support. This position involves working closely with participants to encourage lifestyle changes, track progress, and connect them with necessary resources.
Position Requirements:
High school diploma or equivalent; additional education in health-related fields is a plus.
Certification as a Community Health Worker is preferred.
Strong interpersonal and communication skills.
Ability to work independently and as part of a team.
Experience in community outreach, health education, or related fields.
Willingness to travel

Community Health Worker/Peer Navigator

New Pathways Project/Housing and Justice-Related Services

Position Description:
The Community Health Worker/Peer Navigator (CHWPN) is a program within New Pathways Project (NPP) and reports to the NPP Case Manager Supervisor. Although NPP is located in North Philadelphia, outreach and engagement activities are provided throughout the city. NPP offers a treatment readiness and recovery support program. Services include pre-treatment collaboration for individuals not currently accessing substance use or other related behavioral health services as well as recovery support for those actively engaged in treatment and for those who have completed treatment who can benefit from on-going, peer driven support.
This position is funded through the PA Department of Health for a 2-year period beginning September 2024-August 2026. The CHWPN will have primary responsibility for community-based outreach, engagement, and implementation of harm reduction activities for program participants.
The CHWPN’s responsibilities will be to support, promote and implement harm reduction and prevention services for individuals with opioid use disorder. Activities will include outreach into high-risk communities, engagement of opioid users and assess for harm reduction interventions as well as other social determinants of health, creating linkages between program participants to treatment readiness services at NPP, community-based licensed treatment providers, primary care services and education and engagement of family and community members in opioid overdose prevention and intervention. It is highly preferred that applicants have experience with working with substance-involved individuals and/or have lived experience with substance use.
Position Requirements:
A minimum of three years of experience in public health or social service-related work.
At least three years experience working with diverse populations, low-income individuals, individuals from different religious backgrounds.
At least three years experience conducting community health education programs.
Associate degree preferred; high school diploma at a minimum
Strong oral and written communication skills.
Bilingual skill is a plus.
Shared life experiences in the communities they serve.

Community Health Worker/Specialized Health Services

Position Description:
A Community Health Worker is sought to support a Complex Care Management Team through an integrated approach to care management and community outreach. The Community Health Worker will report to the Sr. Director of Behavioral Health Integration and the Director of Social Services and will be responsible for the identification and engagement of high-risk Health Network patients in the community. The position will be responsible for connecting identified patients to the Complex Care Management Team and to initiating targeted outreach in Philadelphia to patients that have had a lapse in care, especially persons with specified chronic conditions.
The Community Health Worker will also work closely with staff across six PHMC FQHC sites and with stakeholders in the community to engage and link patients to Heath Network services. The position will additionally be responsible for assisting patients in understanding and advocating for their health needs, and in navigating and accessing health and social services relevant to their primary care. The full-time Community Health Worker position will require frequent, local travel and may include home visits and program screenings, where appropriate. It may also include escorting patients to appointments, as necessary.
Position Requirements:
Excellent time management skills and attention to detail to attend to program target goals and to ensure timely documentation
At least two years of experience with vulnerable populations, including incarcerated, substance use and/or mental health disorders; knowledge of Philadelphia is a plus
Ability to adapt to fast paced medical environments and demonstrate flexibility in operating within different work and community settings
Knowledge of medical (especially chronic diseases) and behavioral health conditions
Excellent interpersonal, organizational, and oral/written communication skills
Ability to represent the agency and facilitate a relationship with the community and other providers
Ability to maintain client confidentiality
